2. Home address, email address and telephone number

3. Any medical details ( At the time of treatment)

We require this information for the following reasons:

1. To be able to keep in contact with you regarding appointments

2. To assess any contraindications prior to treatment.

We are committed to ensuring that your information is secure. In order to prevent unauthorised access or disclosure, we have put in place suitable physical, electrical and managerial procedures to safeguard and secure the information we collect. We will never distribute your personnel information to any other service or company unless we have your written permission or are required by law to do so.

In accordance with GDPR you now have certain rights regarding the information that we may have on our system:

Right to access – You will now have the right to obtain information from the data as to how and for what purpose your data is being used for. Furthermore should you ask for it, we will provide a copy free of charge.

Right to be forgotten - Also known as data erasure, you will now have the right to have the data erased if you no longer wish for us to keep your information on file. Client’s files will be kept for seven years. Any files containing your personal and confidential information will be disposed of securely.
